Voluntary Retiree Dental/Vision Plan Options:

The East Syracuse Minoa Central School District is pleased to make available to you voluntary retiree dental and vision plans.
The dental plan, administered by Ameritas, offers either a high or low plan option, each with a $1,500 annual maximum benefit. The vision plan also provides either a high or low plan option, with a broad array of covered services through Davis Vision.
Enclosed are information sheets defining the specifics of both the Dental and the Vision Plans as well as instructions on how to find dental and vision providers. Enrollment Forms and Payment Verification Forms are enclosed.
Coverage is based on a rolling enrollment process. If Enrollment Forms are received by the 5th of the month, coverage will be effective the first day of the following month. To enroll in either or both plans, please review, sign, and return the completed Enrollment Forms AND the Payment Verification Forms to:
